Free Lunch
If a student qualifies for free lunch, we ask that a separate form be used for each child in the household. Forms can then be turned in without payment.
Reduced Lunch
If a student qualifies for free lunch, we ask that a separate form be used for each child in the household. Forms can then be turned in with separate checks for the appropriate amount based on the chart for reduced lunch.
Full Cost Lunch
The full cost of lunch is $2.90 each day. If a student does not qualify for free or reduced lunch, we ask that a separate form be used for each child in the household. Forms can then be turned in with separate checks for the appropriate amount.

If a doctor has determined that your child has a disability, and the disability would prevent the child from eating the regular school meal, FCMA will make any substitution prescribed by a licensed physician at no extra charge. The physician's statement, including prescribed diet and/or substitution, must be submitted to the food service department.
